Overview The Housing Care Navigator supports individuals experiencing homelessness by providing compassionate, trauma-informed care to help them achieve long-term housing stability. This role focuses on assessments, individualized housing plans, advocacy, and connecting participants with vital community resources. You will play a key part in empowering participants to overcome barriers, secure housing, and thrive. What You’ll Do Conduct intakes and needs assessments for participants. Create individualized housing plans tailored to member goals. Provide trauma-informed, harm-reduction support and crisis intervention. Coordinate with landlords, service providers, and community partners. Assist participants in securing identification and housing documentation. Advocate for tenant rights and educate members on lease responsibilities. Facilitate workshops, peer support groups, and orientation sessions. Maintain accurate case notes and documentation in the Homeless Information Management System (HMIS). Meet consistently with members to address barriers and support housing goals. Transport or accompany participants as needed for housing search and resource access. Minimum Qualifications Bachelor’s degree in Human Services, Social Work, Psychology, or related field OR equivalent combination of education and related experience. At least 1 year of experience working with individuals experiencing homelessness and/or behavioral health needs. Fingerprint clearance card (or ability to obtain within 8 weeks). TB test clearance (upon hire and annually). Valid driver’s license and acceptable driving record. Must pass pre-employment drug screening. Skills & Abilities Strong interpersonal and communication skills. Experience working with diverse populations. Skilled in crisis intervention, de-escalation, and trauma-informed care. Proficient in Microsoft Office, HMIS, and EHR/EMR platforms. Ability to manage a caseload in a fast-paced environment. Team-oriented yet able to work independently. Why Join Copa Health?
