Legacy Hospice Services provides adults, children and their families with high-quality, compassionate home-based care in the Portland metropolitan area and communities throughout the Willamette Valley.
Legacy Hospice also provides care in residential or assisted living settings, adult foster homes and nursing care facilities.
ResponsibilitiesThe RN performs professional community health nursing visits and services for terminally ill patients and their families. Provides pain and symptom management appropriate to the patient needs. Member of the Hospice Inter-Disciplinary Team (IDT), coordinates/ assists in the implementation of the plan of care for the terminal patient and their family/caregiver. Completes nursing assessment and provides palliative treatment and symptom management. Responsibilities may include making initial visits to admit patient, initiating and contributing to the revision of the hospice plan of care. Preparation of timely clinical documentation. Participation in IDT and case conferencing as appropriate and contributing to the coordination of services to the patient.
QualificationsEducation : As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.
Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing residency program that is equivalent in content, curriculum and duration.
Hospice experience or one year medical/surgical acute care or long-term skilled nursing experience preferred.
Knowledge/Skills:
Working knowledge of and the ability to apply the basic principles and techniques of community hospice nursing in a variety of home settings.
Must possess excellent clinical nursing skills and be able to utilize problem-solving techniques.
Ability to establish and maintain effective working relationships with professional groups, volunteers and the general public.
Must possess excellent communication skills both verbal and in writing.
Keyboard skills and ability to navigate electronic systems applicable to job functions.
