Reporting to the Practice Manager, and/or Clinical Manager/Leader, and practicing under the supervision of a RN, ARNP, licensed Physician or Physician Assistant, the Licensed Practical Nurse delivers patient care including assessment, treatment, care planning, and medical care evaluation. Completion of a Licensed Practical Nursing program from an accredited School of Nursing is required. Licensure required: New Hampshire Board of Nursing license. Certification required: Current American Heart Association Basic Life Support for Healthcare Providers certification or Red Cross CPR/AED for Professional Rescuers and Health Care Providers. Demonstrated ability to effectively and positively interact with a multi-disciplinary staff, patients and others. Responsibilities include triaging patient phone calls following established guidelines and prescribed format for collecting data and identifying patient's health problems or concerns; documenting relevant health information, interventions, and outcomes into the patient electronic medical record; providing health education and teaching to patients in response to identified patient needs and to promote, attain and maintain the optimum health level of patients; administering prescribed medications according to five rights of medication administration; performing both physical and psychosocial assessments based on patient presentation and complaint of problem and planning nursing accordingly; completing practice and unit specific clinical skill competencies on an annual basis; performing other nursing services commensurate with LPN experience, education and demonstrated competencies. Physical and work requirements include lifting, carrying or pushing/pulling up to 20 pounds occasionally, hearing and speaking regularly, repetitive motion frequently, and occasional bending, climbing, fine motor skills, kneeling, reaching, sitting, smelling, squatting, standing, and walking. Exposure to airborne contaminants, pathogens, bloodborne pathogens, bodily fluids, electrical hazards, and toxic or caustic chemicals is occasional.
